Laser Supported Cosmetic Dentistry

Cosmetic Dentistry

We believe all our treatments need to be aesthetic, even if the treatment is mainly functional. With the latest technology, techniques and materials we usually do not need to compromise from either aesthetics or function. What we mean by “cosmetic treatment” is the treatment we do mainly because of demand of better aesthetic and there are different ways we can achieve that as detailed below. If you have any tooth decay or gum disease, you will need to get this treated before you start cosmetic dental treatment.

Veneers

With the up to date technology and techniques, we can mimic the beauty of natural teeth by very thin shells called veneers. We bond these veneers to the outer surfaces of your teeth to give them their strength back if they have lost or broken tooth structure.

We use veneers to

  • restore a single tooth that may have been fractured or discoloured, or
  • close the spaces between teeth where there is no missing tooth
  • improve the appearance of front teeth while protecting back teeth if they are worn

We can provide multiple veneers to close these spaces, restore the length of teeth that have been shortened by wear, or been chipped, fill the black triangles between teeth caused by gum recession, provide a uniform colour, shape, and symmetry, and make the teeth appear straight.

Veneers have many benefits as they are both aesthetically pleasing and can protect teeth from more invasive dental care procedures while increasing self-confidence and self-esteem.

Advances in ceramic materials and veneering techniques allow us to restore function and aesthetics using conservative and biologically sound methods as well as promoting long term oral health. We consider aesthetics, treatment planning and clinical care in accordance with the interrelationship between the teeth, gums, lips and face because ceramic veneers are primarily indicated for the improvement of smile. We respect the symmetry and the harmonious arrangement of dental-facial elements while designing your new smile.

The choice of materials for veneers depends on your needs. We explain different material options during your aesthetic consultation however there are two major groups of materials we can use. The first one is dental ceramic which presents excellent properties, like stability in colour while having a high capacity to mimic tooth structure and translucency of the natural teeth. The second option is composite materials which are basically tooth colored filling materials that have been highly improved to resemble the visual properties of teeth while maintaining the maximum tooth structure.

Veneer Consultation and Treatment

We will most likely begin with a smile analysis to determine what steps are necessary to achieve the smile you desire. Just as with porcelain veneers, “no-prep” or minimal preparation veneers— so called because they typically don’t require us to remove as much tooth material—are bonded to the front surface of your teeth. Often, the placement of no-prep veneers can be done more quickly and with less discomfort than traditional veneers.

Porcelain laminate veneers consist of a compilation of several thin ceramic layers which replace original tooth enamel, and an adhesive layer. To apply a veneer, a very small amount of the original tooth enamel must be removed, usually less than a millimetre. This preparation creates room for the porcelain veneer to fit within the mouth and most accurately restore natural tooth function while creating an even better appearance than the original tooth.

Having Porcelain Veneers

Porcelain laminate veneers are an extremely thin, translucent cladding that is glued or cemented to the front of teeth to give them a healthy, youthful look. Despite being so thin, the veneers are exceptionally strong and durable and will last a very long time without damage. Because of the advancements in dental veneer technologies, the material will maintain its look and stay in place for anywhere up to 15 years. The other incredible advantage of porcelain veneers is that they do not restrict what you can eat. Their strength and the fact that they are fixed so securely to the teeth because of new bonding technologies usually allows the patient to continue eating all their favourite foods without fear of damaging their veneers.

How are Dental Veneers Applied?

We will have a smile analysis to understand your concerns better and to determine what steps are necessary to achieve the smile you desire. We will take photographs and get copies of your teeth to provide you the best available options for you to choose from, for example we will let you know if you are a good candidate for minimal preparation or “no-prep” veneers and if this option makes a sensible treatment plan.

Whether it is only one tooth or many teeth we design your smile as a whole, which means we integrate every small detail of your natural teeth, gums and face elements to maintain your own characteristics into your veneers. After deciding on a customized treatment plan, our technicians create a model which is called “wax-up”, customized to your individual needs to show you what we can achieve at the end of the treatment before we even touch your teeth.

We will smooth the outer surface of the teeth, taking great care to protect as much enamel as we can, and take impressions either by our intraoral scanner or conventional methods. After agreeing the colour of your teeth, we will send these impressions to our laboratory and our experienced and highly talented technicians will work on your new porcelain laminate veneers. Meantime we will provide you with temporary veneers to protect the surface of your teeth. This also means you can have a chance to try your new appearance and get used to the feeling. In the next session, we bond your porcelain veneers to your teeth after preparing and disinfecting your teeth surface with lasers. Our patients mostly do not need numbing during these procedures. Porcelain laminate veneers may last for about 10 years with proper care. We recommend you to engage in regular hygiene appointments and maintenance examinations to help promote longevity.

When your teeth do not require removal of as much tooth structure we can use no-prep veneers. Just as with porcelain veneers, “no-prep” or minimal preparation veneers are bonded to the front surface of your teeth. Often, the placement of no-prep veneers can be done more quickly and with less discomfort than traditional veneers.

If your preference is composite laminate veneers, then the surface of your teeth will remain as it is but only roughened microscopically to improve the bonding strength or will be prepared with great care to be minimally invasive. In the same session we will work layer by layer to create your veneers in our practice to give you a beautiful smile. This type of veneers may last for 5-7 years and need retouching from time to time as they may get stained especially if you are consuming lots of coffee, tea or red wine.

Bonding

Composite bonding technique is where we use tooth colored filling materials to give a new shape for teeth where there is a small chipped area or small spaces in between teeth. Bonding does not require preparation of the surface of the teeth by drilling so it is a very conservative option for making a big difference with minimal interference.

We ask our patients to have their teeth whitened before this application as the material does not change in colour if it is applied afterwards and this will give us the chance to brighten your smile even more.

crown

Crown and bridges

We rebuild damaged or broken-down teeth using crowns to replace their function, structure and shape while protecting what is left behind from further damage.

We use bridges to replace missing teeth by using the support of the neighboring teeth and placing crowns on top of them to support an artificial tooth in between.

Any tooth under a crown or bridge needs to be healthy and have a strengthened core.

If there is any inflammation in the core of the tooth which needs a root canal treatment prior to the crowns, this is done by laser support to increase the success rate of the root canal treatment. Then the tooth is rebuilt to create a core to support the crown. We occasionally need to place a post to be able to rebuild the core of the tooth by taking support from a root.

If there are decayed tissues on the tooth these are removed by ER:YAG laser to protect as much as healthy tooth structure as possible and to be minimally invasive.

Then we give a suitable shape to the supportive teeth by grinding them down gently as necessary to give the protection it is needed at the end of the treatment. Impressions to create copies of these teeth are either taken by a dental scanner or conventional impressions.

Depending on your need, we can take your impression using our dental scanner and while you are having a short break of a half an hour, we send it to our software, design and tailor it to your needs. Then it is milled at our practice by our milling machine from a material which is suitable for you. With this option, you can have a new crown within 2 hours.

The other option for taking the impression is to use a conventional method where necessary. Then it is sent to our talented dental technicians’ laboratory. We provide you with a temporary crown while your crowns are manufactured. Our dental technicians work on a customized design of your crowns to match the shape and colour of your natural teeth. In this option, you will require two short sessions.

After we try your new crown and make sure that you are comfortable and happy with all aspects of the treatment, we isolate your teeth and place your new restorations to finalize your treatment.

In addition to the natural teeth, crowns and bridges can be built on top of artificial roots called implants. As the implant supported crowns feel more natural, do not add extra work or pressure force to the neighboring teeth and they are proven to be reliable for the long term, we advise our patients to have implant supported crowns and bridges wherever possible to replace missing teeth. We mostly use screw retained crowns and bridges on the implants as the long-term success is higher compared to other types of retention.

To provide the desired aesthetic, function and protection for the teeth, we use up to date materials to create crowns and bridges including precious metal or zirconia supported porcelain to pressed full porcelains. We will explain the materials suitable for your condition in detail during your full examination appointment.

It is possible to mimic neighboring natural teeth with the materials used to give the desired appearance and high standard aesthetics however it may take more time (three to four sessions) especially if it is only one crown in one of your front teeth.

Whitening

Just like our skin and hair, the colour of our teeth can vary, and also become darker naturally. Overtime with frequent consumption of coloured food and drinks or smoking and chewing tobacco, discoloration of teeth may occur more rapidly.

If you are concerned about the colour of your teeth when you talk, smile or laugh, teeth whitening can be a minimally invasive solution for you. You just need an examination to see if your teeth and gums are healthy before beginning your whitening. However if you have multiple fillings in your front teeth, large decays or large gaps between your teeth, then you may need to consult us for different options to improve your smile. We can provide veneers either porcelain or composite or advise you to have orthodontic treatment first for you to have healthier and whiter teeth.

whitening

While changing the colour of your teeth requires whitening, stains on teeth can be removed by our hygienist who will provide you with scaling and polishing. Before whitening applications, your teeth need polishing professionally to be sure your gums are healthy and your teeth stain-free. Then your teeth’s impressions are taken to create plastic trays which fit over your teeth with your whitening gel in the comfort of your own home. We use whitening gels which either contain hydrogen peroxide or carbamide peroxide and when these chemicals are activated by the heat in your mouth, they release oxygen molecules which go into outer layer of your teeth and attach to staining molecules in the teeth. We will explain how to apply your whitening gel while providing your customised whitening package.

The time needed to reach the desired whiteness depends on the shade of your teeth and we will ask for you to visit us to record the new colour of your teeth after your treatments.

You may have temporary sensitivity of your teeth to hot and cold, tender gums, white patches on your gums and a sore throat during and after the whitening procedure which should disappear after a couple of days.

Brushing regularly with a fluoride toothpaste, quitting smoking, consuming less food and drinks

that has potential to discolour teeth, like tea, coffee and red wine and having regular professional cleaning of teeth all help extend the period whitening effect will last.

Pink Aesthetics

Yout teeth’s harmony heavily depends on the gums surrounding them. This may be overlooked especially if it needs extensive surgery to correct. This is one of the main reasons why we use our laser to modify gum levels in addition to most of our cosmetic treatments on teeth. Er:YAG laser provides two main advantages; it can help us adjust gums without using injections for minor adjustments, and it helps us avoid major surgery in most cases; even if the adjustment is quite extensive. Avoiding surgery will decrease/eliminate discomfort after treatment and it will decrease the time needed for the gums to settle to progress with the restoration of teeth. case

Another quite interesting application with laser is that we can “whiten” gums. Whether physiologically or because of smoking if gums have a dark colour either locally or in a wide area we can remove the dark colour without any injections. case

Teeth alignments / straightening

Removable Invisalign Clear Aligners

Invisalign is almost invisible way to straighten your teeth without wearing conventional metal braces. We use a series of clear, removable custom-made aligners to move your teeth with Invisalign. You do not need to worry about food restrictions or difficulties with flossing and toothbrushing as you will remove your aligners before you eat and drink. This technology is also more comfortable, avoiding the mouth abrasions that can be caused by the metal and wires of conventional braces which Invisalign is less time consuming since you will need shorter appointments with us for the adjustments.

After the consultation, we take your impression with intraoral scanner and Invisalign uses 3-D computer imaging technology to design the complete treatment plan. We can see the treatment plan  from the initial teeth positioning to the desired final positioning together with you. About ten days after your impression taking, your aligners will arrive and we will explain you how to use and take care of your Invisalign aligners. You will need to use your aligners between 21-22 hours a day.
